Door hinge assembly orientation

We took apart the door hinges to clean, paint and replace the bushings.  Now I am confised on putting them back together.  3 questions.

1.  The upper hinges appear to be symmetrical.  I have one issue though.  The small cup shaped piece has 2 sides that are different.  One side bends out more than the other.  Which side  goes agains the cowl half and which side goes against the door half?

2. The lower hinges appear to be identical left vs right.  It seems to me that the knurled rollers go up on the driver side and down on the pass side.  Please confirm.

3.  The pin / bushing kit has bushings taht need to have the hinges drilled out to a slightly larger hole before these can be pressed i.  This seems to make sense to me since the holes coould be worn and this would be needed for them to be tight again.  Please confirm.
