3 speed wiper linkage

Hey All,
I bought the kit from Sylvain Gaucher 4 years ago to convert my 2 speed to a 3 speed wiper system. Well, over 4 years, the documentation from Sylvain showing where to drill and install the new bushing has vanished (pun intended). I've emailed Sylvain, but cannot get a response. Does anyone still have the documentation they could share with me?

Geez I feel like a moron. 
I looked at his listing 3 times looking for some documentation before I sent him an email. Scrolled up and down several times. Never once went through his pictures. That's probably why he didn't respond. He was thinking I'm
