clutch pedal rattle

Stupid question, but how in hell do you keep a clutch pedal from rattling !?! I'm referring to where the pedal itself fastens to what I'll call the push rod that has a slot. There is a thin metal washer between the pedal and the rod and everything is held in place with a metal clip. I've spent hours fixing all the other rattles/squeaks, but this one is driving me nuts !!
