Front end alignment

The 4 position Pitman is plausible for sure.

I can see it being 1-2 teeth out of position but no more than that

Sounds like a step by step investigation from the beginning is in order.
Disconnect the pitman arm from linkage.
Turn the wheel full one directions. Turn to opposite counting revolutions. Split that and that will be your "close" to center starting point. Is the steering wheel roughly correct? Or is ti roughly 180 out?
Disconnect the tie rods. Straighten the wheels visually. Now see where the threaded connections of tie rods lie for comparison. If they are still off considerably. Remove pitman arm and check for ability of misalignment as noted above.
Spline is tapered and should be keyed.
Correct if issue is identified.
(Thinking out loud)  could it be possible for Pittman arm to be upside down? Can you post a pic of it installed).
Another "visual" would be where the idler arm angle is ending up. If whole center link is being forced to one side of the other because of pitman steering column misalignment. It would also not be properly positioned.

Pics of their positions with tires (tyres for non US.) straight would answer open questions.
